Replace labels with descriptions. Replace certainty with neutral framing.
Example wording
Examples of neutral phrasing:
• “Based on what we discussed, the main concern was: [client’s words].”
• “I can’t confirm a diagnosis by message — we agreed to: [next step].”
High-risk phrases to avoid
Avoid phrases that sound like diagnosis or medical certainty:
• “This is definitely…”
• “You have…”
• “This is caused by…”
• “You need to do X and it will fix it.”
