2) What was observed or discussed

3) What was explained
Document what was covered in the consultation. No added advice, no assumptions, no promises.
4) What was agreed
Record agreed actions, reminders, or boundaries only if they were explicitly discussed.
5) Follow-up timing
Keep timing simple and factual: when to check in, when to review, or what follow-up point was mentioned.
